I was supporting the air Ops from October 2001 and remember when an ODA team also took friendly fire. That was traced back to a simple mistake using the handheld Laser Designator which, when the batteries were replaced, gave your current GPS co-ordinates until you acknowledged that your GPS location was correct. Only then would it show the targets co-ordinates and in the heat of battle the FAC missed the acknowledgement and thought the GPS co-ordinates the LD was giving him were the targets and called the strike in.
