It is now known that the Flight PS752 of Ukraine International Airlines exploded mid-air before impacting ground in Shahriar, near Tehran, Iran. Cause of the explosion of this Boeing 737-8KV with UR-PSR register is unknown.This video shows it moments before crash

The New York Times has obtained video of the moment a Ukrainian airliner was hit minutes after takeoff from Tehran.

Prime Minister Justin Trudeau says he’s received evidence from intelligence sources suggesting the plane that crashed in Iran on Wednesday, killing at least 63 Canadians, was shot down by a surface-to-air missile.

i would say paranoid incompetence more than an accident. an accident would imply there was no untoward actions but iran was in the middle of attacking their neighbors so "accident' is far to charitable word to use here.I'd say it's more akin to when a bank robber is running from police and ends up crashing their getaway car into someone and killing them.

I totally agree with you, instead of denying the accident, they should have come out and told the truth to the entire world instead of denying it. on top of that, they refusing to hand out the black box too.

How Not to Operate a Surface-to-Air Missile Battery

It’s not yet clear why a Ukrainian jetliner was shot down on Wednesday, but a lot had to go wrong.

It’s getting clearer that the Ukrainian airliner that crashed in Tehran on Wednesday was shot down by an Iranian missile. It’s far from clear whether Iranian forces mistook the plane for an enemy aircraft, shot it down intentionally, or perhaps allowed an air defense system to fire autonomously. 
American officials said Thursday that launch detection satellites and other intelligence showed that Ukraine International Airlines Flight 752 was downed by an Iranian missile minutes after taking off from Imam Khomeini International Airport in Tehran. All 176 people aboard the Boeing 737-800 were killed.
“What’s bizarre about it being a mistake, is that not only was this an airliner that’s operating at medium altitude, it was departing from the nearby airport in Iran leaving the country, not descending into the country from outside of the country,” said David Deptula, a retired U.S. Air Force lieutenant general who was principal attack planner for Operation Desert Storm. “You scratch your head and go, ‘Holy cow, how could this happen?’”
The incident occurred just hours after Iran fired 16 missiles at U.S. forces based in Iraq.

“There are established procedures to achieve positive verification [and] positive identification,” Deptula said. “But remember, when you have humans involved and tensions and anxiety in a combat situation, people make mistakes.”
In 1988, the crew of the Vincennes, a U.S. Navy guided missile cruiser, mistakenly shot down an Iranian airliner flying from Tehran to Dubai. During the 2003 invasion of Iraq, the U.S. military mistakenly shot down a British Tornado jet when a Patriot interceptor mistook the warplane for an anti-radiation missile. And in 2014, Malaysia Airlines Flight 17 was brought down by a Russian Buk missile over Ukraine.
“In one sense [the crash in Tehran is] a bit different; in the other, it’s not,” Deptula said. “There are mistakes that are made particularly in high-pressure, high-tension situations.”
A surface-to-air missile battery operator typically tries to get positive identification of a suspicious aircraft before firing. Since all commercial airliners have a transponder that sends out unique information about that particular aircraft — known as a squawk — the missile operator should check for this info.
“You don’t have to have a radar hit for that,” Deptula said. “That is a device that actually transmits that information.”
If a plane is not transmitting a squawk, the surface-to-air missile operator should check the local flight timetable. If there’s nothing on the flight schedule, he or she should next analyze the flight characteristics of the plane.
“Is this airplane coming in at low altitude and high speed [while] heading toward a sensitive operating area,” Deptula said. “Or is it climbing out on a profile, going through medium altitude at a speed typical of an airliner…and what’s its vector.
“Any one of those three elements, you then have a good sense as to whether this is a hostile aircraft or a friendly or remains an unknown,” he said.
Each country has rules of engagement that dictate when to fire anti-aircraft missiles.
Ukraine Airlines Flight 752 was climbing to the northwest — away from Tehran— and was at 7,925 feet at a speed of 275 knots when its squawk was lost, according to flight tracking data from Flightradar24.
The Defense Intelligence Agency said in its most recent public assessment of Iran’s military said its  “fielding more-capable, domestically developed SAM and radar systems to help fill gaps in its air defenses.” 
A U.S. official told the New York Times that a missile from a SA-15 air defense system shot down the plane. The system can operate in autonomous mode, according to a Missile Defense Advocacy Alliance fact sheet.
A former senior defense official called the incident “totally unacceptable incompetence.” Iran’s air defenses “basically don’t clear anything, they just shoot,” the former official said. 
“We really won’t know [what happened] until there’s an investigation and the people who were involved are debriefed,” Deptula said. “But I doubt we’re going to find that out anyway.”

In stress situation such things can happen, if the fire button is in the hand of trigger happy soldiers without strict control by commanding officers.

Imagine. The persians expected, that the US would strike back after the iranian missile attack. A missinterpretation of radar signals caused the feeling to be under attack. In such situations some guys tend to fire first and ask later.

Shit happens. All over the world.

Its sad for the people in the civil plane. Really sad.

Thats true. some systems which were developed in the late Soviet Union were designed to fight down enemy targets autonomous. Such systems you can find in the Su-22M4, the Ka-50 and Ka-52 for example. Originally it was intended, that the pilot/operator had to designate the target, all other things, including the opening of fire, layed in the "hands" of computers. Today even the target selection and designation can be given into the hands of the computer system. That such systems have a tendency to fire sometimes to fast and without friend or foe analysis we had seen 2 or 3 years ago, when a Ka-52 accidently attacked a truck with exercise spotters during "Sapad" big scale exercise in Russia. The computer took over control, detected a target, decided that the contact was hostile and opend the fire. The crew was surprised. Shit happens.

For me such "accidents" are a clear warning, that autonomous weapon systems should be banned. The last decision over dead or live should be in the hand of man, not in the hand of computers.

The flight of Ukraine Airlines PS752 and it's subsequent downing by Iran's Military is not unfortunate or an accident. An accident would occur if something unexpected resulted from an action. Example: Iran fired a SAM at a military aircraft and it hit a civilian one by accident. In this case they fired that weapon at an aircraft and hit said aircraft. The system did what it was designed (in it's limited capacity it has) to do exactly what it was tasked to do. Was it an accident the civilian aircraft weren't grounded, maybe but I think it's more irresponsible mistake than accident. As for the unfortunate part, the only thing unfortunate in the downing of PS752 is that Iran made an unfortunate denial of the events they soon after had to admit to. 

Iran acted irresponsibly and here's why.

  • Iran did not act to preserve civilian life during a military strike or planned heightened defenses.
  • Iran knew it was going to take military action, the world knew it. 
  • Iran conducted a military strike at the U.S. that was not answered with a counter strike, that is good fortune.
  • Iran fired a SAM and it worked as intended, more good fortune.
  • Iran knew when and what they were doing at the time of the strike.
  • Iran knew they were going to heighten their defensive posture in preparation for a counter strike.
  • Iran knew where the ballistic research facility was located and its proximity to Iran's international airport.
  • Iran knew they were going to protect that facility (they put SAMs there) during such a time as this.
  • Iran knew civilian aircraft frequently traveled the airspace above or near this protected facility as part of their normal flight path on departure (Standard Instrument Departure).
  • Iran did not immediately ground civilian aircraft moments after the Iranian strike began.
  • Iran allowed this aircraft to depart and did nothing to prevent it.
  • Iran did not coordination with air traffic controllers to keep civilian aircraft away from this government installation, either in foresight or hindsight.
  • Iran and the air traffic controllers flew this aircraft into or allowed it to traverse known protected airspace.
  • Iran caused the loss of the passengers, the crew, and the aircraft.
  • Ukrainian Airlines knew the hostility was going on and they allowed this aircraft to depart without grounding it.

It's human nature to want to sympathize and understand the "fog of war", "young inexperienced soldier", "heightened intentions", "shit happens", etc., but those are spins of the truth.

The truth is Iran took military action which resulted in the direct loss of civilian life and Ukrainian Airlines didn't protect its passengers or equipment. It was wise Iran owned up to their actions and inaction but that does not excuse them.

We are all flawed human beings. This was a mistake, Iran wishes they had a do-over, and it is tragic but let's not diminish the lives of those who were lost by explaining it away. May peace come to the families affected and may they find closure with their loss of their loved ones.

Just in

Iranian authorities released the British Ambassador in Tehran after arresting him for a while.

I really don't know what is going on in Iran thing's are getting fast.

No charges can be forced against an Ambassador and to arrest him is illegal/ against the diplomatic immunity unless while doing his crime. Even at that time, authorities have no right to keep him arrested.
