From what I found on M.A.T.S.

F-14A VF-21   18.04.1995 Two VF-21 aviators ejected from their F-14A aboard USS Independence whem the nose wheel slipped off the port side of the flight deck as they were taxiing inot position for launch. Both crew members were rescued uninjured. The F-14 was recovered from the edge with only minor damage.

amazing they survived w/o injuries!!! One of the chutes looked like just barely opening as he apparently hit the water :ok:

Yeah, that barrier wasn't up. Somebody paid big time for that mistake. Heck, maybe a host of somebodies when you think about it.

Doesn't the air boss give clearance to move into position and such? If he gave that thinking it was up, his fault. If the pilot moved there anticipating it, his fault. If the pilot on the cat throttled up too soon, his fault.

Without the full accident report, it's just supposition.

(Is it SOP to eject?)

going over the edge?  Oh Yea!!!!

had they waited and the plane started over they could have been out of the envelope in about .05 picoseconds!!  If one has any doubt, it is time to go!!

yea, one of those really, really bad days on the birdfarm!

USS Independence wow, I heard some of the Forrestal-class carriers did not have blast fences for the waist cats.

Anyone know?

Looks like it's in the description of the original video - no water cooling in JBD 4 (probably 3, too?) so it had to remain down for the F-14's taking off.  I recall an airframe type talking about a different incident where the JBD "failed," which led to a similar outcome.
